Provider Information

The Provider List contains all providers associated with your practice. This is where you keep their schedules, their identification numbers and other information needed for claims, and their contact information. Enter all providers and hygienists in this screen.

 

Quick Steps

1.

Click Setup in the toolbar.

 

2.

Locate the Practice Information section and click Set up providers.

 

3.

Press F8 or click New Provider.

 

4.

Create your own code or let the program create the code for you based on the provider's last name.

 

5.

Enter the provider's name and information in the correct boxes, selecting a specialty if applicable.

 

6.

In the Appointment Color box, choose a color to identify this provider's appointments at a glance in the Scheduler.

 

7.

If claims need to be billed through the name of another provider, choose that provider.

 

8.

Choose the type of "signature" you want to appear on your claims in the Print in Claim Signature Box field.

 

9.

Open the Work Schedule tab.

 

10.

Enter the provider's normal working hours.

 

11.

Open the IDs tab.

 

12.

Enter the provider's various assigned IDs in the appropriate boxes.

 

13.

Click Save Changes.

 

Repeat these steps for each dentist and hygienist in your practice.

 

Full Instructions

Go to the Lists menu and select Provider List. Or click Setup in the toolbar, locate the Practice Information section and click Set up providers.

The Provider List shows all providers and hygienists already entered in the program. Each provider and hygienist who is part of the practice needs to be entered here.

 

To create a new provider record, click New Provider in the Options bar.

The Provider Entry screen has three tabs in which to record important information.

 

Provider Information tab

In this tab you enter demographic information, indicate the provider's specialty, assign an identifying color to identify this provider's appointments in the Scheduler, and make some selections concerning claim information.

 

You can leave the Code field empty and let the program create a unique code or create one yourself.

 

Enter all demographic information.

 

Enter the provider's Credentials.

 

Choose the specialty. There are options here for hygienist. The specialty codes are paired with corresponding HIPAA codes that are automatically populated in box 58 of the 2003 ADA claim form and box 1 of the 1999 ADA claim form.

 

Choose an appointment color to identify this person's appointments in the Scheduler. This is the color of the bar on the left of the appointment.

Often hygienist's claims are sent using a provider's name and ID number. If you are entering hygienist Smile N. Calm and want her claims to be sent under Dr. Ur D. Best's name, select Dr. Best's name in the Bill Claims using another Provider box. His information is shown in box 48 of the 2003 ADA claim form.

 

If the provider you need has not yet been set up in the program, click the field and press F8 to create the new provider's record "on the fly." Create the needed record and save it. You'll be returned to this screen and this record, and the record you just created on the fly is selected in this box.

 

In the Print in Claim Signature Box, select what you want printed on the claim: "Signature on File" or the provider's name.

 

Work Schedule tab

Open the Work Schedule tab.

Establish this person's regular work hours. If an appointment is scheduled for this person outside his or her indicated hours, the program alerts you to this fact and asks if you want to override the regular hours.

IDs tab

Open the IDs tab.

In the IDs tab, you enter the provider's assigned IDs needed to file claims.

 

The Office Number field is an extra field for identifying numbers. If an insurance company gives you a special identification number, we recommend that you use the Provider Number field first, then the Office Number field.

 

The State ID Number is the provider's state-issued license number.

 

The TIN Number (or EIN Number) is provided by the government.

 

The Drug ID Number is your DEA# which authorizes you to dispense prescriptions.

 

NPI stands for "National Provider Identifier," a new standard in uniquely identifying providers. This is part of new HIPAA regulations. If you have not received an NPI for your provider, leave this field blank.

 

The Provider Number is provided for any other number you need to use. If an insurance company gives you a special identification number, enter it in this field.

 

The other fields are self explanatory.

 

Be sure to click Save Changes or press F3 to save the record.

 

Customizing

The columns displayed in the Provider List screen are customizable. See Customizing a Screen.

 

In addition, the filters can also be changed -- added to, removed, etc. See Customizing Filters.
